First Visit
Once you have scheduled your first appointment, please complete your health history and other registration forms online and securely submit them to the office before you arrive. Simply, click on the PATIENT REGISTRATION tab above or Click Here to Download our Patient Registration Form . This will save you valuable time and help us prepare for your appointment. Please make every effort to arrive on time. Dr.'s Kirsch, Jacobs & Sandau take great pride in being on-time for scheduled appointments.
If you have been referred to the NJ Center for Oral Surgery, please bring any x-rays or referral slips that have been given to you. Do not worry if you do not have x-rays. Depending on your condition, we may need to take a specialized x-ray called a Panorex.
At your consultation you will meet privately with the doctor who will review your medical history and go over the reason for your visit. After the doctors examination, they will discuss your treatment options. This appointment will last however long you need to make sure you fully understand your condition and choices.
Certain oral surgery procedures are covered under insurance. Dr.'s Kirsch, Sandau & Jacobs participate with several insurance plans and their team is happy to help you. You must bring all your dental insurance information as well as photo identification to your first visit. After your consultation appointment their experienced insurance department can contact your insurance to determine your benefits for the planned procedure.
